Self-Management Assembly
Health and wellbeing must be less about ‘doing to’ people and more about ‘working with’ people to better manage their own lives.
Contact with health professionals represents a fraction of a person’s care experience when living with a long-term health condition. Self-management helps people draw on their own strengths and abilities to manage or minimise the ways in which that health condition can limit their life and general wellbeing. But motivation is not enough; we all need confidence and knowledge to succeed.
ASSEMBLE is the self-management assembly for West Dunbartonshire which will
- Promote health literacy and improve opportunities for people to lead their care and support
- Understand the wishes and needs of local people and work to increase the range of options to help them live well in the community
- Create mutually beneficial relationships between people living with long-term conditions and service providers
- Encourage transformation and innovation in prevention, early-intervention and community capacity focussed on health and wellbeing outcomes.
The ASSEMBLE initiative will deliver:
- 2 self-management assembly resource open days per year
- Themed networking events (virtual and in-person)
- On-line resource directory – available (Coming Soon).
The first Self-Management Assembly took place in March 2025 with 24 national health support charities in attendance.
